Aftercare Planning with Your Treatment Team
As you prepare to discharge from Laguna Treatment Hospital, your case manager will help you work out all the details.
Laguna’s medical staff will evaluate your physical and mental health, as well as your progress in addiction therapy to develop an aftercare program that fits your needs. This may consist of an intensive outpatient program (IOP), partial hospitalization program (PHP), or another form of outpatient care. Continuing your care on an outpatient basis is critical to your continued recovery, and you will know your full schedule of appointments before leaving Laguna so you don’t miss a beat.
If you need continued medication to ease cravings or stabilize your health, this will be arranged by your doctor and you will have all the details before you leave the hospital.
Your case manager will be in touch with you every step of the way, checking in to make sure you’re attending your outpatient sessions, going to group, and taking your prescriptions. Your case manager is also your main point of contact if you have any questions about your aftercare plan or need to switch outpatient providers for any reason. They will even help place you in a transitional sober living environment if needed.
At Laguna Treatment Hospital, many of our case managers are alumni of Laguna and in recovery themselves. They understand how difficult it can be to transition into a life of sobriety, and this compassion drives them to serve your needs as best they can to help you succeed.
Staying Connected in Recovery
As you begin your new life in recovery, staying in touch with your case manager will help you stay connected to Laguna’s extensive support network. They can refer you to doctors and therapists, help you find an outpatient program that’s in-network with your insurance provider, and help you connect with a local 12-step group.
Your case manager is not your only support network. Laguna Treatment Hospital’s alumni meet regularly to connect with each other, discuss the challenges and victories they experience in recovery, and connect with each other.
Many of our alumni have found housing, jobs, and a new circle of sober friends to spend their weekends with just by participating in alumni events.
